You must apply for the assessment in writing or in person at the relevant office. Depending on the services offered by the office responsible for you, you can also apply online.
You can obtain the form there or on the website of the Stuttgart Regional Council. You can also download an application form and an information sheet there. If you submit the application in person, the responsible office will prepare a transcript.
Tip: Fill out the application as completely and legibly as possible. The district office, the city or municipal administration, the welfare office for war victims or the integration office will help you with the application. If special circumstances arise after the application has been submitted (for example dismissal)you should inform the District Office immediately.
The office will analyse the medical documents you have submitted and, if necessary, will also obtain further medical reports from the doctors treating you if you have signed the release from medical confidentiality on the application form.
You will then receive an assessment notice from the District Office - even if the GdB determined is less than 50. The assessment notice contains
- the individual disabilities,
- the degree of disability (GdB) and
- the other health characteristics (markings).
If the assessed GdB is 50 or more, you will receive a severely disabled person's pass.
